Home Health Heart Care
Once discharged from the Cardiac Rehabilitation program, the NPMC Home Heart Care team serves as a bridge from the outpatient program into independent living. The program helps instruct patients on diet and promotes the goals set while in Cardiac Rehab.

Other services provided by the Home Heart Care team include:
• Chest Pain Management
• Monitoring Vital Signs
• Cardiopulmonary Status
• Cardiovascular Status
• Monitor medication compliance
Importantly, the Home Heart Care team can routinely check for abnormalities or problems if they are to occur. Patients can utilize therapy services provided by Home Health in order to help enhance cardiac recovery.
