Skilled Nursing Facility
The Skilled Nursing Facility or “SNF,” is a special unit designed to assist patients in regaining and maintaining their previous level of activity and function that they had prior to becoming ill.
SNF provides additional care for patients who no longer need the type of care required during hospitalization, but who are not yet physically ready to return home to self-care. Through this program, patients may gain strength with the help of a skilled multidisciplinary team of Nurses, Physical Therapists, Dietitians, Social Services, Activity staff and Pharmacists. With the efforts of the team, family and the patient, an individualized treatment is used in order to strengthen the patient to return home or to continue on to Rehab.
